The Red Cliff Community Health Center (RCCHC) now offers secure online access to your doctor’s office and personal medical records here at the Red Cliff Community Health Center. A personal health record enables you to manage your health information, remain informed and communicate more effectively and easily with your health care providers.
With a Personal Health Record (PHR) you can:
- Schedule an appointment.
- View portions of your medical records including test results, current medications,
current diagnosis, list of allergies and immunization history. - Send a message to your provider and more…
There two ways to become a Personal Health Record (PHR) user:
- Ask to sign up for PHR during your next Clinic visit. Completion of an identify verification form is required.
(or) - Go to the PHR website and select “Register” to create your account. Before you can see or access your medical records in PHR, you must verify your identity in person at the Red Cliff Community Health Center.
Example forms of identification include:
Driver’s License
Tribal Identification Card
Employment Identification Card/Badge
Military Identification Card
Passport
Never use My Personal Health Record electronic communications in an emergency.
In Case of an Emergency or for Urgent Medical Matters call 911
RCCHC staff will respond to your Personal Health Record (PHR) phone message or email within 24 hours or the next business day.
